على جميع المتقدمين تسليم عطاء المناقصة باليد أو بالبريد السريع أو بالبريد المسجل مع الإيصال قبل موعد إقفال المناقصة والمحدد الساعة 2 بعد الظهر من يوم 17 فبرايـر 2008.  

All PPSC Bidders must make sure that the Tender  is hand-delivered, sent by courier, or sent by registered mail with proof of receipt, or delivered by hand against receipt to the designated address no later than the date and time specified in the invitation to Tender (14:00 hrs on 17 February 2008)?

على مقدمي العطاء التأكد من ختم الأظرف بالشمع الأحمر دون ذكر أى بيانات على الأظرف عدا كلمة "العرض الفنى" أو "العرض المالى" والبيانات المذكورة بالفقرة التالية أدناه مطبوعة بالآلة الكاتبة.

على مقدمي العطاء التأكد من تقديم العرض الفني والعرض المالي كل في أظرف منفصلة.

All PPSC Bidders must make sure that the Technical Proposal and the Financial Proposal are submitted in separate envelopes.

على مقدمي العطاء إعداد وتقديم كتاب رسمي(يسمى لاحقا كتاب تقديم العطاء أو كتاب التقديم) يرفق ضمن العرض الفني و أن يكون موجه إلى رئيس الأمانة العامة لنقطة الارتباط الوطنية الكويتية متضمناً البيانات من 1 إلى 11 المذكورة أدناه:

All PPSC Bidders must prepare and include a cover letter (hereinafter called the “Proposal Submission Letter”) inside the envelope of the Technical Proposal addressed to the Secretary-General of the Secretariat of Kuwait NFP that includes the information requested in items 1 to 11 below:

(1) اسم مقدم العطاء (الجهة/ الشركة / مجموعة الشركات المتحـدة).

1.  Name of the Tenderer (Entity/Firm/Consortium)

(2) في حال تقديم العرض من مجموعة شركات متحدة ، أسماء جميع أعضاء المجموعة وتحديد العضو الرئيسي فيها؟

2.  If a Consortium, the names of the other

     members of the Consortium and the leading

     member from among the Consortium members.

(3)   أسماء وأرقام تلفونات وعناوين الشخص أو الأشخاص المخولين بالاتصال.

3.  Names and contact details of person(s)  

      responsible for contact.

(4)  اسم محدد للوكيل المحلي وصفته القانونية بالجهة/الشركة/مجموعة الشركات المتحدة.

4. Name of the local representative and its legal

    Status within the Entity/Firm/Consortium.

(5)  بيان خطي من الجهة / الشركة/مجموعة الشركات المتحدة، مرفق ضمن كتاب التقديم  يؤكد بأن المناقصة تم إعدادها وتقديمها حسب ما هو مطلوب في:

i.        إرشادات المناقصة (إنجليزي)

ii.      الشروط العامـة (إنجليزي/عربي)

iii.    الشروط المرجعية (إنجليزي)

5. A statement from the Tenderer (Entity/Firm/

   Consortium) included in or attached to the

   Proposal Submission    Letter confirming that the

   Tender response has been submitted in

   compliance with (i) the Instructions to the

   Tenderers (English); (ii) the General Terms and

   Conditions (English and Arabic) and (iii)  the

    Request for Proposal (RFP) and Terms of

    Reference (ToR) (English)?

(6) بيان خطي من الجهة / الشركة/مجموعة الشركات المتحدة مرفق مع كتاب التقديم أو فقره ضمن كتاب التقديم  ، تؤكد بأن مقدم العطاء يتحمل نتائج أي خطأ ناتج عن عدم الإلتزام  باي بند من بنود شروط تقديم عروض المناقصة.

6. A statement from the Tenderer

    (Entity/Firm/Consortium) included in or attached      to  the Proposal Submission Letter stating that

    the Tenderer shall bear full responsibility for any

    error resulting from NOT complying with any 

    item in the Tender Submission Rules?

(7) بيان خطي مرفق مع كتاب التقديم أو فقرة ضمن كتاب التقديم تشير بأن العرض الفني يشتمل على مستندات التأمين الأولي وهي مليون دينار كويتي -/1,000,000د.ك. ( مليون دينار كويتي) مقدمة بصورة ضمان بنكي باسم" الأمانة العامة للجنة المركزية" صادر من أحد البنوك الكويتية.

7. A statement from the Tenderer included in or

    attached to or  the Proposal Submission Letter

    confirming that Preliminary Insurance issued by

    a Kuwaiti Bank has been taken out in the form of

    a bank guarantee drawn in favor of the

    “Secretary-General, Central Committee” for

    KD. 1,000,000 (one Million Kuwaiti Dinars)?

(8)  فقرة ضمن كتاب التقديم تفيد بأن التوقيع على العرض الفنى والعرض المالى للمناقصة وكتاب التقديم قد تم من قبل مقدم العطاء أو الوكيل المحلي المخول بالتوقيع.

8.  A statement included in the Proposal

    Submission Letter the Technical and Financial

    proposals as well as the Proposal Submission

    Letter are signed by the Tenderer or his/her duly

    authorized local representative?

(9) فقره ضمن كتاب التقديم تفيد بوجود نسخة أصلية من العـرض المـالي ضمن ظرف العرض المالى.

9.  A statement included in the Proposal

    Submission Letter  indicating that there is one

    (1) original hard copy of the Financial Proposal.

(10) فقرة ضمن كتاب التقديم تفيد بوجود ديسك ممغنط على نظام (Read only) تحتوي على نسخة واحدة كاملة من العرض المالي وملحقاته على أن يكون على نظام (Format PDF) ضمن ظرف العرض المالى

10. A statement in the Proposal Submission Letter

      confirming that there is one (1) Compact Disk

     (CD) in Read Only Format containing the

    Financial Proposal submission along with the

    attachments in PDF format and included in the

    envelope of the Financial Proposal.

(11) على مقدم العطاء أن يذكر كتابةً أنه قام بتعبئة وتوقيع وختم نماذج الإقرار الواردة في المادة (27) من الشـروط العامة باللغة العربية والإنجليزية وإضافتها ضمن العرض المالى.

11.  The Tenderer must indicate in the submission

       letter that he has filled out, signed and stamped

       the Arabic and English Declaration Forms

       attached to the General Terms and Conditions

       as Article 27 (see Document 2 of the Tender

       Dossier) and included the signed form within

       the Financial Proposal.

التأكد من تقديم العرض الفنى والمالى باللغة الإنجليزية.

All PPSC Bidders must ensure that the Technical and Financial proposals are submitted in the English language.

التأكد من أن العطاء يشتمل على مستند التأمين الأولي بقيمة -/1,000,000 د.ك. (مليون دينار كويتي) بصورة ضمان بنكي باسم "الأمانة العامة للجنة المركزية" صادر من أحد البنوك الكويتية صالح لمدة 90 يوم ومرفق ضمن ظرف العرض الفنى.

All PPSC Bidders must make sure that the Preliminary Insurance of KD. 1,000,000 in the form of a bank guarantee from an approved bank in Kuwait to the order of the “Secretary-General, Central Committee” valid for 90 days is included in the same envelope as the Technical Proposal.
